AGED CARE

The Adelaide Northern Division of General Practice Aged Care Program has a key role in the implementation and operation of the Aged Care GP Panels Initiative. The Aged Care Program team participates in the following:
  • undertaking local liaison, development and administrative support work for the Panels Initiative
  • work with RACFs in their area to develop local responses to barriers
  • work with GPs, allied health service providers and RACFs to determine activities to address needs at a local level
  • promote interest among GPs, allied health service providers in providing services to residents of RACFs
  • facilitate the implementation and operation of the Panels Initiative and undertake related activities to achieve the Panels Initiative’s aims, and
  • share information across the Division network through the AGPN

Medical care of older persons in residential aged care facilities  Silver Book 4th edition “aims to provide general practitioners, and other health professionals including residential aged care nurses with a resource for delivering quality health care in residential care facilities. The overall content was developed and reviewed by a national taskforce for health professionals working across the aged care sector. In addition, general practitioners, consumer representatives and aged care experts provided valuable input and feedback”. …..”This publication encourages collaboration between health professionals and provides suggestions for implementing systematic care involving residents, their general practitioners, residential aged care facility staff, families and other carers”. The Australian Government recognizes that “residents of aged care facilities are amongst the sickest and frailest of Australians, often having complex needs requiring multidisciplinary providers. Some residents of residential aged care facilities (RACFs) experience difficulties in keeping the services of GPs and allied health service providers who could provide regular consultations for residents. This is especially the case where a resident does not have a regular doctor…”
