Rural Palliative Care Program
Adelaide Northern Division of General Practice has received a 2 year grant to implement a Rural Palliative Care Program in the Gawler region.
The key objectives of the program are;
- To provide a high quality and responsive palliative care service to patients and carers in the Gawler Health Service District.
- To ensure that appropriate care and expertise is available when needed.
- To prevent inappropriate use of services such as Ed or hospital services when better alternatives are available.
- To use active multi disciplinary care planning and case conferencing to permit coordinated services, anticipated patient problems and respond to patients preferences such as place of care
- To make this service available to all that need it within the community.
- To help to facilitate a seamless and easy pathway of service for patients and their carers.
Adelaide Northern Division look forward to working with the community and stakeholders in order to improve accessibility to quality, coordinated palliative care in the Northern area.
Specialist Palliative Care services for the Northern area
Lyell Mc Ewin Palliative Care Services
This service is available for people living with a life limiting illness, their families and carers.
It provides support and care, including medical care, nursing, health and counseling.

Modbury Public Hospital palliative Care Service
Service provide for people living with a life limiting illness and their families and carers.
Referral required from health practitioner 12 bed unit providing medical, nursing, allied health care, counseling &day care centre activities.

Children and Adolescent Services
Service provided Phone support and advice for General Practitioners, families and Patients who are living with a life limiting illness.
Home visits and counseling support for families, patients and General Practitioners.
Covers the area of the whole of South Australia and the Northern Territory.
